میزان شیوع ژن مقاومت mec A در سویه های استافیلوکوکوس اورئوس جدا شده از نمونه های بالینی بیمارستان ها و مراکز درمانی

<div style="text-align: justify;"><span style="color:#000080;"><strong>سابقه و هدف:</strong></span> باکتری استافیلوکوکوس اورئوس به عنوان یک عامل شایع عفونت ­های بیمارستانی به شمار می رود. سویههای مقاوم این باکتری یک پاتوژن مهم و عمده در ایجاد بیماری و مرگ و میر در ایران و جهان میباشد. لذا هدف از این تحقیق، بررسی میزان شیوع ژن مقاومت به متی سیلین mecA و تعیین الگوی مقاومت آنتی بیوتیکی در سویه های استافیلوکوکوس اورئوس جدا شده از نمونه های بالینی می باشد.<br>
<span style="color:#000080;"><strong>مواد و روشها: </strong></span>در این مطالعه مقطعی - توصیفی 100 جدایه استافیلوکوکوس اورئوس از بیمارستان­ ها و مراکز درمانی شهر تبریز جمع آوری گردید . این جدایه ها با استفاده از روش های استاندارد آزمایشگاهی و کشت در محیط اختصاصی تعیین هویت و وارد مطالعه گردیدند. تعیین هویت سویه های MRSA به روش فنوتیپی و مولکولی انجام شد. همچنین جهت بررسی فراوانی ژن­ mecA از روش PCR استفاده شد. به منظور ارزیابی الگوی حساسیت آنتی بیوتیکی سویه ها، از روش انتشار دیسک بر اساس پروتکل CLSI انجام گردید.<br>
<span style="color:#000080;"><strong>یافتهها:</strong></span> از 100 نمونه مورد بررسی، 75 نمونه استافیلوکوکوس اورئوس مقاوم به متی سیلین بودند. ارزیابی فنوتیپی الگوی مقاومت آنتی بیوتیکی سویه های استافیلوکوکوس اورئوس مقاوم به متی سیلین نشان داد که بیشترین مقاومت نسبت به آنتی بیوتیک­های پنی سیلین به میزان 100% و کواموکسی کلاو 91/66% بود. بررسی مولکولی نشان دهنده حضور 68% ژن mecA در ایزوله­ ها بود.<br>
<span style="color:#000080;"><strong>نتیجهگیری: </strong></span>شیوع سویه های استافیلوکوکوس اورئوس مقاوم به متی سیلین در این مطالعه بالا بوده است. همچنین مطالعه اخیر نشان دهنده افزایش مقاومت استافیلوکوکوس اورئوس مقاوم به متی سیلین نسبت به سایر آنتی بیوتیک های مختلف بوده که این مسئله یک هشدار جدی جهت درمان عفونت های ناشی از استافیلوکوکوس اورئوس در منطقه می باشد. بنابراین به منظور جلوگیری از افزایش مقاومت نسبت به سایر آنتی بیوتیک ها، ضروری می باشد که از تجویز بی رویه و غیر ضروری آنتی بیوتیک های در دسترس خوداری شود.</div>
<div style="text-align: justify;"><span style="color:#000080;"><strong>Background and Objective:</strong></span> Staphylococcus aureus bacteria is a common cause of hospital infections. Resistant strains of this bacterium are a major pathogen in causing disease and death in Iran and the world. Therefore, the aim of this study was to investigate the prevalence of mecA methicillin resistance gene and determine antibiotic resistance patterns in S. aureus strains isolated from clinical specimens.<br>
<span style="color:#000080;"><strong>Materials and Methods:</strong></span> In this descriptive cross-sectional study, 100 strains of S.aureus from Tabriz hospitals and treatment centers were collected. The isolates were identified by standard laboratory methods and cultured in a specific Culture Media. The identification of MRSA strains was done by phenotypic and molecular methods. The PCR method was used to determine the frequency of mecA gene. In order to evaluate antibiotic susceptibility patterns of strains, a disk diffusion method based on CLSI protocol was performed.<br>
<span style="color:#000080;"><strong>Results:</strong></span> Of the 100 samples examined, 75 samples of methicillin-resistant S.aureus were found. The phenotypic evaluation of the antibiotic resistance pattern of methicillin-resistant S.aureus showed that the highest resistance to penicillin antibiotics was 100% and Co-Amoxiclav was 91.66%. Molecular examination revealed the presence of 68% of the mecA gene in isolates.<br>
<span style="color:#000080;"><strong>Conclusion:</strong></span> In this study the prevalence of the antibiotic resistance pattern of methicillin-resistant S.aureu was high. This recent study suggests increased resistance to methicillin-resistant S.aureus in compare to other antibiotics, which is a serious warning for the treatment of S.aureus infection in the region. Therefore, in order to prevent the increase of resistance to other antibiotics, it is essential to avoid the administration of uncontrolled and unnecessary antibiotics.</div>
