fa روند جهانی و نابرابری‌های درآمدی در کم‌وزنی هنگام تولد: یک تحلیل اکولوژیک و پیش‌بینی تا سال ۲۰۳۰ Global Trend and Income Inequalities in Low Birth Weight: An Ecological Analysis and Projections to 2030 آمار زیستی و اپیدمیولوژی Biostatistic & Epidemiology مقاله پژوهشي Research Article سابقه و هدف: کم‌وزنی هنگام تولد (Low Birth Weight یا LBW) منعکس‌کننده شرایط سلامت مادر، مراقبت‌های دوران بارداری و وضعیت اجتماعی-اقتصادی جوامع است. ارزیابی روند جهانی این شاخص همراه با نابرابری‌های درآمدی می‌تواند بینش مناسبی از الگوی LBW در سال‌های اخیر فراهم کند. این مطالعه با هدف تحلیل روندهای جهانی LBW طی سال‌های ۲۰۰۰ تا ۲۰۲۰، بررسی نابرابری‌های درآمدی و پیش‌بینی وضعیت تا سال ۲۰۳۰ انجام شد. مواد و روش ها: در این مطالعه اکولوژیک، داده‌های شیوع LBW از پایگاه داده WHO برای ۱۸۰ کشور طی سال‌های ۲۰۲۰-۲۰۰۰ استخراج و بر اساس طبقه‌بندی درآمدی بانک جهانی تحلیل شد. برای مقایسه میانگین LBW بین گروه‌های درآمدی از آزمون ANOVA استفاده گردید. با توجه به ساختار طولی داده‌ها، مدل رگرسیون با اثرات تصادفی (Random Intercept) با کشور به‌عنوان اثر تصادفی و متغیرهای سال، سطح درآمد و منطقه جغرافیایی به‌عنوان اثرات ثابت برازش شد.  برای دسته‌بندی کشورها بر اساس سرعت تغییرات، تحلیل خوشه‌بندی K-means با استفاده از میانگین تغییرات سالانه استانداردشده LBW انجام و تعداد خوشه‌ها با روش Elbow تعیین گردید. پیش‌بینی تا ۲۰۳۰ با برون‌یابی خطی روند هر گروه درآمدی انجام شد. تحلیل‌ها با Stata 17 و سطح معناداری 05/0 انجام شد. نتایج: میانگین جهانی LBW طی دو دهه مورد بررسی از ۶۷/۱۱ درصد در سال ۲۰۰۰ به ۲۳/۱۱ درصد در سال ۲۰۲۰ کاهش یافت. شکاف معناداری بین گروه‌های درآمدی وجود داشت (۰۰۱/۰P>)؛ کشورهای کم‌درآمد بالاترین میزان (۵۸/۱۵ درصد) و کشورهای پردرآمد پایین‌ترین میزان (۲۶/۸ درصد) را داشتند. ویتنام (۴/۳۶- درصد)، افغانستان (۶/۲۵- درصد) و سنگال (۳/۲۱- درصد) بیشترین کاهش و کویت (۶/۴۲+ درصد) و اردن (۵/۲۹+ درصد) بیشترین افزایش را تجربه کردند. سه خوشه متمایز شامل کشورهای با تغییرات آرام (۹۴ کشور)، کاهش سریع (۵۴ کشور) و افزایش سریع (۳۲ کشور) شناسایی شد. برون‌یابی خطی روند سالانه هر گروه درآمدی تا سال ۲۰۳۰ نشان‌دهنده تداوم شکاف درآمدی است؛ به‌طوری که میزان LBW در کشورهای کم‌درآمد به ۷۴/۱۳ درصد و در کشورهای پردرآمد به ۶۶/۸ درصد خواهد رسید (P<0.001). نتیجه گیری: علیرغم کاهش آرام میانگین جهانی LBW، نابرابری عمیق و پایداری بین کشورهای با سطوح مختلف درآمدی وجود دارد. دستیابی به اهداف توسعه پایدار نیازمند مداخلات هدفمند با تأکید بر کاهش نابرابری‌های ساختاری و تبادل تجارب موفق بین‌کشوری است.   Background and Objectives: Low birth weight (LBW) reflects maternal health conditions, prenatal care, and the socioeconomic status of nations. Evaluating the global trend of this indicator along with income inequalities can provide valuable insights into the pattern of LBW in recent years. This study aimed to analyze global LBW trends from 2000 to 2020, examine income inequalities, and project the situation to 2030. Materials and Methods: In this ecological study, LBW prevalence data were extracted from the World Health Organization database for 180 countries during the period 2000–2020 and analyzed based on the World Bank four‑group income classification. One‑way analysis of variance (ANOVA) was used to compare mean LBW rates across income groups. Multivariate linear regression was applied to identify key macro‑level determinants (income level and geographic region). K‑means clustering analysis was employed to classify countries according to their rate of change over time. Projections to 2030 were made using linear extrapolation of observed trends. All analyses were performed using Stata version 17, and a significance level of less than 0.05 was considered statistically significant. Results: The global mean LBW decreased from 11.67% in 2000 to 11.23% in 2020 over the two decades studied. There was a significant gap between income groups (P < 0.001); low-income countries had the highest rate (15.58%) and high-income countries had the lowest rate (8.26%). Vietnam (−36.4%), Afghanistan (−25.6%), and Senegal (−21.3%) experienced the greatest decreases, while Kuwait (+42.6%) and Jordan (+29.5%) showed the largest increases. Three distinct clusters were identified: countries with slow changes (94 countries), rapid decreases (54 countries), and rapid increases (32 countries). Linear extrapolation of the annual trend for each income group to 2030 indicates the persistence of the income gap, with LBW rates reaching 13.74% in low-income countries and 8.66% in high-income countries (P < 0.001). Conclusion: Despite a slow decline in the global mean LBW, a deep and persistent inequality exists between countries with different income levels.
