Abstract
Aim and Background: Considering the importance of the birth experience and its psychological consequences, and the limited and contradictory evidence regarding its role in postnatal attachment, the present study aimed to determine the relationship between birth experience and maternal postnatal attachment in primiparous women.
Materials and Methods: This cross-sectional study was conducted on 268 primiparous women hospitalized in the postpartum ward of Shahid Gholipour Bukan Hospital in 2023. Sampling was done by simple random method. Data collection tools included demographic information form, childbirth experience questionnaire, and maternal postnatal attachment scale. Data analysis was performed using Pearson correlation coefficient tests and univariate and multivariate linear regression in STATA14, considering the normality of data distribution. p≤0.05 was considered significant.
Results: The average age of the participants was 25.41 years. The average and standard deviation of the childbirth experience score was (2.2±0.24), with the dimensions of perceived safety, maternal participation, and staff support having the highest and lowest scores, respectively. The average and standard deviation of the adjusted postpartum attachment score was (3.46±0.24), with the highest scores related to enjoyment in interaction and the lowest to attachment quality. Among the dimensions of birth experience, the dimensions of empowerment (p=0.018) and perceived safety (p=0.005) had a significant relationship with maternal attachment after birth. Having more income than expenses (p=0.01), no history of hospitalization during pregnancy and postpartum (p=0.008), skin-to-skin contact immediately after delivery (p=0.02), spouse support (p=0.03), and childbirth experience score (p=0.05) had a significant relationship with maternal postnatal attachment.
Conclusion: The findings emphasize the importance of improving obstetric care in improving the birth experience and maternal postnatal attachment; therefore, it is suggested that future studies with interventional and longitudinal designs examine the role of dimensions of the birth experience and mediating variables in the formation of mother-infant attachment
